Once you've finished work on a construction project and gotten the final payment, it's time to provide the owner with this final Conditional Waiver and Release of Lien.

• This form is for work, materials or equipment supplied to job sites in the State of Washington.
• The contractor or supplier waives and releases all of its lien rights, bond claims, and other claims for payment, but only on the condition that the final payment is properly endorsed and clears the bank on which it's drawn.
• The contractor warrants that all invoices for materials and services for the project have been or will be paid from the final payment.
