This breach is to be used ONLY for a breach of the rental agreement arising from unauthorized persons residing in the premises, an unauthorized pet, or damage / destruction to the rental premises.

The tenant is required to remedy the breach within 3 days or the landlord will terminate the rental agreement. A Service of Notice form is included, to record the details of service.
