Consent of Individual to Use of Name

Use this template to prepare a Consent Form, to be given by the principal of an existing company consenting to the use of his / her name as part of the name of a new company which is being incorporated. The form also discloses that the individual has a material interest in the new company. This is a generic corporate form in MS Word format, and is not specific to any country or region.
