Photographer Model Agreement and Release Form for Minors

Photographers, have a parent or guardian of an underage model sign this Model Agreement and Release for Minors form prior to a photo shoot.

• Purpose. The purpose of this Agreement and Release is to protect your rights to the photographs you take.
