Use this template to prepare an Affidavit of Records, which would be sworn by a party to a legal action in Province of Alberta Court of Queen's Bench with respect to records and documents which form part of the evidence in the lawsuit. The Affidavit sets out:

• the records (documents) which the party has in its possession,
  
• the documents it objects to producing,
  
• the documents it no longer has in its possession.
