Film producers, use this template to prepare Box Kit Rental Agreement / Vehicle Allowance forms for employees and contractors being hired to work on the film production. This form is to be attached as a schedule to the Deal Memo between the parties.

The form lists the tools and equipment belonging to the employee which are being rented by the production company and will be used by the employee during the shoot. The employee will be paid a specified amount per day or week for using his/her own vehicle on the production.
