Appoint an attorney to manage your finances and property in the event that you can no longer do so, with this Continuing Power of Attorney for Property for the Province of Ontario, in accordance with the Substitute Decisions Act, 1992. This document allows you to appoint someone you trust to make decisions on your behalf for issues relating to your property only. It does NOT grant authority for health care or personal care decisions.

You can authorize your attorney to act in respect of all of your property or only such property as you specify. The attorney may also have conditions and restrictions imposed on its powers as detailed in the document. You can choose if the power of attorney will come into effect only if you become mentally incapable or if you wish to have it become effective immediately. The form includes instructions for completion, and information about making a power of attorney.
