Contract Change Order
Use this template to prepare a Contract Change Order which must be used to approve changes to the terms of the original contract between a customer and a contractor with respect to a construction or renovation project.
Changes requiring a Change Order would include additions, amendments or deletions to the scope of the work, the materials, extras, etc, with corresponding adjustments to the contract price. Both the customer and the contractor are required to sign the form in order to authorize the changes.
