The forms would be used in connection with a contractor who is being hired on the production as a loan-out (i.e., loaned out to the film production by the contractor's own company as an independent contractor, and not as an employee of the production company).

The corporate waiver is given by the contractor as required by Canada Customs & Revenue Agency, declaring that he / she is not an employee of the production company and instructing the payroll department that no taxes or other amounts are to be deducted or withheld from the loan-out's contract fees.
