Use this template to create a Notice to Tenant of Tenancy Termination for the State of Colorado for tenancies of six months or more but less than one year. The notice must be served by a landlord or property manager on a tenant of a rental unit.

The notice requires the tenant to vacate the premises by a specified time and date. This form is only to be used to terminate a tenancy where the tenant has lived in the rental premises for a period of at least 6 months, but less than 1 year. Also included is a Service of Notice form.
