Use this template to create a Notice to Tenant of Tenancy Termination for Non-renewal of Tenancy for the State of Utah, to be served on a tenant by a landlord or property manager of a rental unit.

The notice advises the tenant that the rental agreement will not be renewed, and requires the tenant to vacate the premises by a specified date or the landlord has the right to commence eviction procedures. This form of termination notice is for fixed term tenancies (such as 6 months, 1 year, etc). Also included is a Service of Notice form.
