Use this template to create a Notice to Tenant to Vacate for Breach of Rental Agreement for the State of Utah, to be served on a tenant by a landlord or property manager of a rental unit. This form is to be used for any breach of the agreement OTHER THAN for non-payment of rent, causing a nuisance, unauthorized subletting, damaging property, or the tenant being involved with drugs.

The notice requires the tenant to vacate the rental premises within 3 days or the landlord has the right to commence eviction procedures. Also included is a Service of Notice form.
