Use this template to create a Rental Unit Vacancy Report to be completed by a resident manager or landlord / agent detailing:

• any loss or damage caused by the tenant.
  
• the costs of repair or replacement.
  
• any unpaid rent or other amounts owed by the tenant.
  

The form includes a Notice of Refund for the balance of the damage deposit, and a Notice of Amounts Owing for any outstanding balance still owed by the tenant after application of the damage deposit.
