Use this template to prepare a School Field Trip Permission Slip to be signed by a parent or guardian allowing a student to participate in a field trip or other event which involves traveling away from the school. The form includes the following provisions:

• The parent gives permission for the school to secure medical treatment for the student if necessary, in the event that the parent cannot be contacted.


• The parent indemnifies and holds harmless the school, the teachers, the school district and related parties against any liability for injury or death arising from the student's participation.


• The school will provide alternate activities for students who do not participate.


• Students must travel on the transportation provided and be picked up and dropped off at the school, unless alternate arrangements have been agreed to.
