Use this template to create a Verified Statement of Lien form in Alabama, pursuant to Ala. Code § 35-11-213, which is a sworn statement given by a contractor, subcontractor or other party making a lien claim against certain property for which the person giving the statement has provided labor and/or materials and has not been paid by the owner. The Statement sets out the details of the claim and information about the property and the owner, and must be filed with the Office of the Probate Judge in the county in which the property is situated.
