- This form can only be served after you have served an initial Notice to Vacate.
- The landlord notifies the tenants that because they have not paid the rent as set out in the initial Notice, they must vacate the rental premises.
- If the tenants do not vacate the premises as demanded, a Sheriff may evict the tenant.
