Professional pet sitters must always be prepared to handle any situation that may arise. To ensure the safety and well-being of the pets in your care, it is important to have clients complete an Emergency Veterinary Care Authorization & Release form for each pet they leave with you.
Authorization to Obtain Veterinary Care
This form grants the pet sitter permission to contact the veterinarian specified by the client if the pet requires medical attention. In circumstances where the client's veterinarian is not available, the pet sitter is also authorized to seek alternate veterinary care as needed.
Veterinarian Authorization
The client's veterinarian is permitted to diagnose and treat the animal as necessary. There is an optional clause included in the form that allows the client to set a maximum limit on the cost of any such treatment.
Client Responsibilities
By completing this form, the client acknowledges that they bear sole responsibility for all costs and expenses related to their pet's veterinary care. Additionally, the client certifies that their pet is current on all required vaccinations.
Release of Liability
The client also agrees to release the pet sitter from any liability related to the care and treatment of their pet.
