Scope of Use

This form is intended exclusively for use with rental properties outside the jurisdiction of New York City. A different form must be used for tenancies within the City of New York.

Notification and Termination Period

The tenant is hereby notified that their month-to-month tenancy will be terminated effective one month from the date of service of this notice. The tenant is required to vacate the premises by the end of this one-month period.

Landlord's Rights and Next Steps

If the tenant does not voluntarily vacate the property by the specified date, the landlord reserves the right to take further action as permitted by law to recover possession of the premises.

A Service of Notice form is included with this document. This accompanying form should be used to record the details of how and when the Notice of Termination was delivered to the tenant, ensuring proper documentation of service.
