The release and waiver of lien rights by the subcontractor (supplier) is limited to the time period up to the date specified in the Partial Waiver.

This is the statutory form required by S. 570.115.9(d) of the Act.
The waiver is only effective on the condition that the contractor actually receives payment from the owner, lessee or designee for labor, materials or equipment supplied for improvements to the owner's property.
