Ohio contractors, provide the owner with this Unconditional Waiver and Release form when the progress payment check has cleared the bank.

• Under Ohio law, contractors must release lien rights within 30 days of receiving a progress payment.
• The contractor warrants to the property owner that all of the laborers, suppliers and subtrades have been paid and that there are no outstanding invoices for the period covered by the progress payment.
