Parties

This Agreement is entered into between two parties: (i) a woman who wishes to conceive a child via artificial insemination, and (ii) a man who voluntarily agrees to donate his sperm for the insemination procedure.

Parental Rights

Child Support

The mother expressly waives any right to claim child support—whether financial, emotional, or otherwise—from the sperm donor.

Paternity Suits

The parties agree that no father will be listed on the birth certificate. Additionally, both agree not to disclose the identity of the sperm donor as the natural father of the child.
