This is the statutory form required by S. 570.115.9(d) of the Act.
The waiver is only effective on the condition that the contractor actually receives payment from the owner, lessee or designee for labor, materials or equipment supplied for improvements to the owner's property.
Only use this form for final payment, not for progress payments.
The law prohibits anyone from requesting a waiver in advance of the work being done or the materials being provided.
