  • There are two separate forms to download. One is the Statement of Earnings form and the other is the Instructions for completing the Statement.
  • A Form 40B must be completed each year by all official staff, temporary and combined-position court reports, except for those serving on a contractual basis or from time to time on a "when-actually-employed" basis.
  • The form must be completed and submitted prior to April 15 in each earnings year.
